What medical information do we require ?

• Blood Type ( If known )
• Medical allergies 

• Food Allergies

• Existing Medical Conditions

• Regular medication that you are taking (names of drugs/medicines.)

Also, if you have any other special medical needs, please feel free to inform us so we can make the necessary arrangements. 

List of Documents required

• Passport Copy
• Overseas Medical Insurance copy
• Confirmed flight ticket copy
• Recent Photograph
• Visa/ eVisa Travel Authorisation

For Indian Nationals : Any one of document with Government approved Photo ID ( Voter ID card, Aadhaar Card and PAN Card ) 

Note : PAN not required if age below 14
